The meeting was called to order.  Adrian Bishop presented Locked Cottage Plans for cottages 4A, 4B, 4C and 4D, in accordance with Guideline 67, Guidelines for the Use of Locks in Cottages.  Carol Badran also presented locked cottage plans for cottages 1A and 1D.

The committee went into Executive Session.  The normally scheduled programs were reviewed.  The committee went into Open Session.  The programs reviewed were approved.  The minutes of the previous meeting were approved. The minutes of the special LHRC meeting were approved. The locked cottage and electronic monitoring form was updated and signed.  Physical management plans were signed for 00490, 00641, 0324, 00444, 00398, 00720, 275214, 00745, 00797, and 00309.

The critical incidents were read and included the following:

A resident fell out of the bed while getting up during the night to use the bathroom. He sustained a laceration to the left eyebrow which required sutures. He is on 1 to 1 status at night to help prevent falls and a mat with an alarm that goes off when he gets up is being ordered.

 

A resident became upset when he could not have fried chicken for lunch. He threw a box at staff and turned around and tripped falling against the wall. He sustained a laceration to the right eyebrow which required steri-strips. He is being followed by the psychiatrist due to frequent increased agitation.

 

A resident was putting the vacuum away at the work site and lost her balance. She caught herself with her right arm on the wall. She sustained a distal radial head fracture to the right hand. She is being followed by the orthopedist for treatment.

 

A resident was observed to have a reddened area to his eye. He was taken to the ER for physician evaluation and diagnosed with conjunctivitis and a corneal abrasion. The injury resolved without complication.

 

A staff member accidentally closed the linen closet door on a resident’s fingers. This caused a laceration to the third and fourth finger on the left hand. Sutures were applied and the areas have healed.

 

A resident was asked to participate in his routine hygiene program and the resident then engaged in self injurious behaviors including falling to the floor and on the walls and falling out of his chair. Bleeding was noted in his mouth and staff discovered he had a loose tooth.  The tooth was extracted by the dentist. A plan is in place for SIB which staff will continue to follow.

The advocate distributed his report.  He told the committee that the Parent’s Group have been reminded that the SHRC and the LHRC may be opportunities for them to address their concerns.

A special meeting was called.  A subcommittee was appointed to hear the discussion.  Adrian Bishop, APM cluster 4 and 2D asked that the committee meet to hear a request for additional mechanical strap to the physical management plan for resident 00324.  He is a 70 year old gentleman who will jump up from a chair or bed and walk very hurriedly to the bathroom.  His gait is unsteady and he has had many falls as a result.  Ms. Bishop asked that a secondary strap be placed over the current seat belt to prevent him from rising without assistance.  A pressure mat will be placed on his bed to notify staff members that his torso has raised off the mattress so they can assist him out of bed.

Dave Long, APM of cluster 3 and 2A asked that a secondary strap be applied to the chair of 00741 as well.  This too, will prevent her from leaving her chair without the staff assistance she needs to be safe.

Both plans were signed and will be discussed with the full committee in the next regularly scheduled meeting.
