The meeting was called to order.

The committee went into Executive Session for the purpose of addressing agenda related items and approval of programs. 

The normally scheduled programs were reviewed.

The committee went into Open Session. A motion was made and seconded to approve the programs reviewed during the meeting.  A motion was made and seconded to approve the minutes of the last meeting.  

The critical incident report information was not available at the time of the meeting but has since been gathered.  A resident fell on the sidewalk and sustained a laceration to her forehead. Five stitches were applied.  Another resident stood up from the table and lost his balance and fell backwards causing a laceration to the back of his head. Four staples were applied.  A third resident pushed another resident causing him to bump his head on the entertainment center. Four staples were applied to the laceration.  A resident was pushed by his peer and he fell into the treadmill sustaining a laceration to the right fifth finger. Dermabond was applied to the area.  A different resident was aggressive towards property by kicking repeatedly at the file cabinet. He sustained a fracture of the medial tibial plateau. He is being followed by the orthopedist.  Finally, a resident fell while he attempted to get his toy car back from his peer. He sustained a laceration over the right eyebrow. Four stitches were applied.

One abuse investigation was initiated during the timeframe. Final results are pending

The advocate read his bi-monthly report.  See the attached report.

The next meeting will be held on July 21, 2008 at 8:00 am in the Staff Training conference room.  There being no further business the meeting was adjourned.
