LOCAL HUMAN RIGHTS COMMITTEE

July 21, 2008

The meeting was called to order.

Since the committee did not have quorum a subcommittee was established to addressing agenda related items and review of programs.  

The normally scheduled programs were reviewed.

There were two abuse investigation initiated in June.  One has been substantiated and we’re still waiting for Richmond to concur on the recommendation of abuse for the second one.  There has not been an investigation started thus far in July.

The critical incidents for June are as follows:

A.  A man received a laceration to his forehead from bumping into an entertainment center.  He was in his room going through his roommates belongings, when his roommate pushed him. Staff will continue to redirect him when he attempts to go through his roommate’s belongings.

B. A man sustained a laceration to the left posterior side of his head.  He stood up from the dining room table and lost his balance and fell backwards hitting his head on the corner of a desk.  The injury resolved without any complications.  He has had an increasingly unsteady gait. The physicians are currently evaluating his medications to see if it could be medication related.

C. An incident occurred that involved a man receiving a laceration to his right fifth finger.  He was pushed and fell into a treadmill by his peer.  The area was dermabonded. The injury healed without complications. His peer has a behavior management plan in place. Staff will continue to monitor him.

D. A man was displaying aggressive behavior that included kicking a file cabinet.  Staff later noticed him to be limping on his right leg.  X-rays indicted a fracture. He is being followed by the orthopedist. His behavior management plan will continue to be followed.

E. A man received a laceration to his right eyebrow from bumping into a wall.  He stated one of his peers took his toy car.  It appears that he fell against the wall when he attempted to retrieve his toy car form his peer.  The injury has healed with out complications.  He was started on a new medication and has developed increasing unsteadiness. The medication was discontinued.

F. A staff member noted a man’s right big toe to be bruised. He stated he had bumped his toe on his bed.  The injury is resolving. He is being followed by the orthopedist.

The advocate read his bi-monthly report.  See the attached report.

The next meeting will be held on August 18, 2008 at 8:00 am in the Staff Training conference room.  There being no further business the meeting was adjourned.
