Social Services

Social workers provide the primary point of contact between SEVTC’s interdisciplinary teams and the Community Services Boards (CSB) with overall responsibility for each Center resident. Social workers manage the pre-admission process and provide the team, resident, and family with information and guidance at the time of admission. Throughout each resident’s stay, social workers assist the team in developing services that target the goals and interests expressed by residents, parents/Legal Authorized Representatives (LAR), and CSBs. Social workers maintain relationships with families and LAR’s and ensure that their questions and concerns are adequately addressed. The social worker regularly updates the CSB on each resident’s status and assists in the development of discharge plans. When the interdisciplinary team, the resident and family/LAR, and the CSB agree that an individual is ready for discharge, the social worker is actively engaged in the placement process in order to achieve the best outcome possible.
